School Accident Program (SAP)
The District provides coverage for school accidents through Philadelphia Indemnity Insurance Company. In the event that a student is injured on District property during school hours or during school sponsored and supervised activities (including athletics), the nurse, school secretary, coach, or other staff who may have WITNESSED the incident should be notified in order for a Accident Claim form to be started and sent home. Parents / Guardians need to complete part ll of this form, sign/date it and send it to the address located on top of the form. All charges should be submitted to your primary medical or dental insurance first.
