Health Records (Proof of Immunization)

All students must provide proof of immunization within fourteen days of the student’s first day of school. The requirement to show proof of immunization is extended to no more than thirty days for any student transferring from out-of-state or another country or on a case by case basis.
Proof of immunization must be any 1 of the 3 items listed below:- An immunization certificate signed by your healthcare provider
- Immunization Registry report (NYSIIS or CIR from NYC) from your healthcare provider or your county health department
- A blood test (titer) lab report that proves your child is immune to the diseases that New York requires all students to be vaccinated against
For varicella (chickenpox): A note from your healthcare provider (MD, NP, PA) stating your child had the disease is also acceptable.
