MOVING AND CHANGES OF ADDRESS
In case of an emergency, it is imperative that we have accurate contact information. Files must be immediately updated reflecting new phone numbers and addresses. Help us insure your child’s safety by updating this vital information. All address or contact information changes are to be reported to the grade appropriate House Office and require proof of address such as: rent receipt, phone bill, utility bill, or bank statement, etc.
