Transition at the CSE Meeting
Once a student turns 15 they are required, per New York State regulations, to be invited to their Committee on Special Education meeting and conversations surrounding transition will occur. Transition conversations and planning should address the student's plans after high school, look at whether or not coursework is on track for graduation, and review services needed to be ready for post-secondary goals.
Students are encouraged to take a leading role during the CSE meeting. Students should be encouraged to share and elaborate on their post-secondary plans and goals. If the student can’t attend, the team must make sure the student’s interests, needs, and goals are considered and documented in the Individual Education Plan and Age Appropriate Career Assessment.
In regards to the student's transition and transition planning, please review the following at the meeting:
- Student transcript
- Progress towards graduation
- Discussions about any potential safety net options or appeals that may be considered
- Any additional accommodations or supports that are needed to assist the student in meeting their post-secondary goals
- Current Age-Appropriate Transition Assessment and tool
- Student Exit Summary, if applicable
