HIPAA/FERPA
The Rochester City School District requires consent under HIPAA and FERPA to exchange student health information and educational records between healthcare providers and district staff. Under FERPA regulations, educational records, including school nursing records, evaluations, grades, disciplinary actions, and Individualized Education Plans (IEPs), as well as protected health details, will not be disclosed without consent. Completing this authorization allows designated physicians to share medical and behavioral health information with school personnel to support the student's health needs and maintain a safe, appropriate, and least restrictive educational environment.
